Very possible, I am working on such a thing right now (for conferencing reasons), but I am using the person's webcam, and directx for the individual users. Just need to take a picture, send the picture, take a picture, send a picture
works good on lan, sucks on the net if you want decent fps. Ah, I havent figured out audio transferring and capturing from a mic, yet.

with your equipment you are able to automacally translate your voice in any other language on the fly, while watching any DVD in a high precision on your 14" display. good luck with on the fly compressionsvideo chat and voice chat

I tried to do a search on the net for something like this, but it seems it's very hard to find somethingn usefull...
I guess most commercial software doing this relies on internal algorithms made in-house...
So you'd better build your own!!!
hehe..
but seriously.. perhaps if you do a more extensive search than I did, you might come across some technical documents or if you're lucky, a .dll that you can use...
[edit]
here's one of the commercial packages:
http://www.nctsoft.com/products/NCTVideoStudio/
.. there's bound to be alot more out there, but they'll probably be expensive..
