At first this was an icon designer but it seems to combine images.
Handy especially since it uses bitmaps however it does lack ico support.

I do not want to hack this post but here you'll find iconbrowser, a tool of mine..

It will let you read an ico file and you can obtain one or more icons from the ico file to combine those to a new ico file.
Even png icons but those are not shown under XP.
It's just an alternative.
