In one game I played, I started in madagaska but the virus never got off the island. maybe that's just an issue with relaxed mode

You have to have only the mildest symptom and rats or airborne (I had both for a while...) and be extremely patient... let it spread but not kill anyone...
hen save your points... buy up lethality... SELL anything that you don't want visible....
then whammo... buy heartfailure and vomiting and hemmoraging... all forms of transmission...

Which is silly... Rat borne will go with Ships, Insects will be blown into new areas by storms... Flooding will help spread waterborne... and Airborne would theoretically cover the planet in 18 hours via winds!

(I think) Airborne refer to ones that can be transmitted by just coughing and sneezing etc. There's a couple of references here regarding influenza http://en.wikipedia.org/wiki/Introduction_to_virus
