2. Install MSDN.

What he meant with "special access to the whole MSDN if I install it" if I still have to place the help file in the help directory as normally (i.e. not special) I don't know.

It created a folder called "Microsoft Platform SDK for Windows Server 2003 SP1" in my start menu.
Ok, I found out what the problem was. To get it to work I needed to open the "Platform SDK Documentation" once. Then it started working.

I'm downloading the ISO image of it from here...

...with a view to burning an install CD, but it has an *.IMG extension and not *.ISO,
so I'm assuming Nero will be able to burn it; but I'm not sure because when I select
"Disc Image" there's no mention of *.IMG anywhere... maybe all I'll need to do is
rename it to have an *.ISO extension? I better not be wasting my bandwidth!

Apprently, it's a standard ISO image. Microsoft; like many others; like to muddy the waters and rename things. I have various tools that can read and convert between various formats and will let you know tomorrow if you have any trouble.
