1. Create a new file with the debugger turned on.
2. Open Compiler -> Options.
3. Uncheck "debugger".
4. Click cancel.

Now, the debugger is turned off in the menu and on the toolbar, but if you open the compiler options the debugger is turned on.

I don't do much nowadays but try to reproduce bugs
But anyway, this one is reproducable.. I think it lies on some wrong validation of the Cancel button..
